Information you choose to send
The appointment form asks for your name, phone number, optional email, preferred date, general concern and consent to be contacted. Do not submit Aadhaar numbers, payment card details, passwords or unrelated sensitive information.
How it is used
Form details are emailed to the clinic for the purpose of responding to an appointment request. The website does not create an online patient account and does not intentionally store form submissions in a website database.
Security and limits
The form uses validation, a session security token and spam protection. No internet transmission is completely risk-free. Medical emergencies and urgent clinical information should never be sent through this form.
Third-party services
The contact page embeds Google Maps, which may receive technical information such as your IP address under Google’s own policies. WhatsApp, email and telephone links open the respective service or application you choose to use.
Server logs
The hosting provider may keep routine security and access logs, such as IP address, browser type, requested page and time of access. Their retention is controlled through the Hostinger hosting account.
